Care Compass Network Welcomes New Partners

Care Compass Network is continuing to grow and involve new partner organizations in our PPS’ work through the Delivery System Reform Incentive Payment (DSRIP) program. We are pleased to welcome these new organizations to join us as we work collectively towards a common goal; transformation health care for the Medicaid population.

Children’s Home of Wyoming Conference

Children’s Home of Wyoming Conference is dedicated to the safety, permanency, and well being of children, their families, and the community. They offer four areas of assistance to children and families: Residential, Educational, Preventive and Therapeutic Care. These programs have been created to inspire home, develop skills, and cultivate healthy relationships that lead towards positive futures of the children, their families, and the community.

Gadabout Transportation Services, Inc
In 1976 Gadabout Transportation Services was founded by a group of people and agencies that saw a gap in transportation services for residents of Tompkins County. Gadabout now owns 31 lift-equipped vehicles to provide safe, reliable, and affordable transportation services to locations throughout Tompkins County for the elderly and disabled residents. To be eligible for transportation services from Gadabout, an individual must be 60 years of age or older. However, if an individual is under the age of 60 and has a disability that interferes with their ability to use public transportation, they can contact the office to see if they can be eligible for transportation services.

Glove House, Inc
Glove House, Inc is  not-for-profit agency that serves youth and families from the Southern Tier and Finger Lakes regions of New York State, empowering them to live successfully within their neighborhoods. Many of the children that are in the care of Glove House have experienced multiple crisis and trauma in their lives, including abuse and neglect. Glove House operates numerous group homes and provide foster care, preventive and school-based services that help youth become productive and successful adults.

Steuben Senior Services Fund, Inc
Most elderly individuals would choose to stay in their homes for as long as possible. Steuben Senior Services Fund (SSSF), Inc is a not-for-profit senior citizen charitable fund that was established to supplement future public funding for the aging. SSSF looks to provide alternative, more appropriate and cost effective ways to meet the basic in-home needs of the elderly population residing in Steuben County.

Tompkins County Office for the Aging
The Office for the Aging in Tompkins County provides information on the services available for older adults and their caregivers to help assist them to live independently in their homes. The Office for the Aging can also help connect elderly individuals with meal delivery services, health insurance, agencies that can assist with home repairs, in-home help, legal assistance, and transportation services.
