CCN Welcomes New Partners

Care Compass Network is continuing to grow and involve new partner organizations in our PPS’ work through the Delivery System Reform Incentive Payment (DSRIP) program. We are pleased to welcome these new organizations to join us as we work collectively towards a common goal; transforming health care.

Green Street Pharmacy (Addison’s Apothecary, Inc)
Located in downtown Ithaca, Green Street Pharmacy has been dedicated to the health of the residents of Tompkins County. They provide free local delivery of prescriptions, have a medication drop-off box, and provide other services such as vaccinations, vitamins, free Wi-Fi, and an old-fashioned soda fountain.

Pro Action of Steuben and Yates, Inc
Pro Action is dedicated to building a community of resilient individuals and families to meet their basic needs, overcome diversity, and prosper. Pro Action offers a comprehensive, integrated suite of services to support their clients. Their multi-generation approach in programs has four key domains: Resilient Children and Families, Thriving Seniors, Prosperous Futures, and Basic Needs Support. Pro Action conducts Community Needs Assessment every three years in order to keep pace with the needs of the community.

Tioga County Department of Mental Hygiene

The Tioga County Department of Mental Hygiene offers residents of Tioga County a comprehensive continuum of counseling services and support for individuals of all ages and families coping with emotional problems, mental health, depression, alcoholism and substance use disorder. The Mental Health Department has two locations in Tioga County; their primary clinic located in Owego and a satellite clinic in Waverly.
