The New York State Department of Health recently released a summary report on the completed MAX series from 2017. Between January and July 2017, 22 Action Teams from across NYS participated in the most recent MAX Series, which focused on improving care for High Utilizer patients. The goal of this series was to reduce inpatient admissions by 10% over a 6-month period. Over the course of the MAX Series, each Action Team developed a High Utilizer care pathway, and measured process improvements to achieve change within their local hospitals and communities.
