New Year, New Partners Join Care Compass Network

Care Compass Network is continuing to grow and involve new partner organizations in our PPS’ work through the Delivery System Reform Incentive Payment (DSRIP) program. We are pleased to welcome these new organizations to join us as we work collectively towards a common goal; transforming healthcare.

Broome County Department of Social Services
The Department of Social Services promotes self-sufficiency and ensures the protection of vulnerable individuals and families. They help connect individuals and families to the resources that they need such as, Family & Children’s services, HEAP, SNAP, Medicaid, Temporary Assistance, and more.

Broome County Mental  Health Department
The Mental Health Department of Broome County is responsible for planning, developing, and evaluating mental hygiene services within Broome County. Mental Hygiene services include alcoholism and substance abuse services, mental health programs, and services for those with developmental disabilities.

Family & Children’s Service of Ithaca
Family & Children’s Service of Ithaca serves over 2,000 individuals and families within Tompkins and surrounding counties. They support, promote and strengthen the well-being of individuals and families by providing high-quality, accessible mental health care and related social services, with a focus on the needs of children. Family & Children’s Service offers some of the following programs and services: mental health and children’s mental health programs, psychiatry, employee assistance program, youth services, and senior services.

Southern Tier Independence Center (STIC)
STIC is a community-based advocacy and service organization for children and adults with all types of disabilities. STIC focuses on providing programs and services that support and promote independence, inclusion, and community integration. They believe that individuals with disabilities have the right to choose where they live and with whom, have the ability to work in a competitive job at a fair wage, plan their own social activities, use public transportation, vote independently in an accessible polling place, and have the adequate supports to live the lives they choose.

Syracuse Recovery Services, Inc
Syracuse Recovery Services are specialists in individualized substance use treatment. They provide counseling services that help individuals make successful progress and get back on their feet as soon as possible. Syracuse Recovery Services offers services such as DMV/DWI evaluations, outpatient services, Medication Assisted Treatment, individual and group counseling sessions, smoking cessation, as well as a Spanish speaking program.

The REACH Project, Inc
The REACH Project is a newly established non-profit organization whose mission is to ensure that People Who Use Drugs (PWUD) have access to compassionate healthcare, regardless of their personal circumstances. REACH Medical, a wholly owned professional corporation of REACH is expected to open their facility on February 28th. Their goal is to provide Opioid Replacement Therapy (ORT) for individuals dependent on opioids, as well as serve as a health hub for this population.

Unity House of Cayuga County
Unity House is dedicated to empowering and enriching the lives of individuals in recovery, coping with a mental illness, and/or diagnosed with a developmental disability. Unity House is headquartered in Auburn, New York and provides services for more than 700 adults a day within Cayuga, Tompkins, Wayne, Onondaga, Seneca, and Ontario counties. Unity House provides person-centered services and programs, such as service coordination, day habilitation, residential support services, and individualized support services.
