The NYS Health Foundation believes patients have a meaningful role as partners in their own health and health care and should be empowered to voice their concerns and ideas. This also means having a voice when it comes to making informed policy decisions.
To support their Patients as Partners initiative, the NYS Health Foundation has launched a Request for Proposals (RFP) to support projects that seek to empower the patient’s voice, provide system improvements, practice innovations, or policy reforms that are designed to benefit patients. Grants of up to $250,000 that will be available to organizations for the Patients as Partners RFP.
All interested applicants must complete an online inquiry form by May 16, 2018 at 1pm. After NYS Health reviews the inquiry forms, select applicants will need to submit a full proposal. Specific instructions will be emailed to the accepted applicants, at which time, they will have until 1pm on July 23, 2018 to submit their full proposal.
