Cardiovascular Disease Management CVD – Project 3.b.i

Overview

Cardiovascular Disease Management ensures clinical practices in the community and ambulatory care settings use evidence-based strategies to improve management of cardiovascular disease. Strategies are focused on improving practitioner population management, adherence to evidence-based clinical treatment guidelines, and the adoption of activities that will increase patient self-efficacy and confidence in self-management.

Care Compass Network’s Goals

  • Provide knowledge and training on Cardiovascular Disease Management.
  • Increase patient self-efficacy and confidence in self-management.

Who is Eligible to Participate in the Project?

Any Primary Care Clinic that is Patient-Centered Medical Home (PCMH) Certified or is in the process of becoming certified or any Community-based Organization that handles Medicaid members who have a Chronic Disease, specifically Cardiovascular Disease (CVD).
