Chronic Disease Management/ Prevention COPD – Project 4.b.i.i

Overview

This project targets Chronic Obstructive Pulmonary Disease (COPD) and how to deliver high-quality preventive care and management to help prevent much of the burden of chronic disease to avoid many related complications. Despite many cost-effective or cost-saving services, many patients do not receive the recommended preventative care and management, including screening tests, counseling, immunizations, or medications used to prevent disease, detect health problems early, and prevent disease progression and complications.

Care Compass Network’s Goals

  • Increase access to quality chronic disease preventive care and management in clinical and community settings.
  • Decrease the number of avoidable complications in Medicaid members and to help lessen the burden of a chronic disease.
  • Provide Medicaid members access to community preventive resources.

Who is Eligible to Participate in the Project?

Any Hospital or Community-based Organization that provides a service(s) to Medicaid members with Care Compass Network’s nine-county region.
