With help from Care Compass Network and United Health Services, the Family & Children’s Society recently expanded their services to include health coaching. This program, which is part of the Delivery System Reform Incentive Payment (DSRIP) program, “coaches” participants following their hospital discharge to prevent avoidable readmissions within 30-days.
Health Coaches work with eligible individuals prior to their discharge from the hospital, working with them to make the transfer from the hospital to their home as smooth as possible. Health Coaches provide services from reviewing medication instructions to helping set up follow-up appointments with Primary Care Providers, helping patients feel informed and lowering anxiety levels related to caring for oneself at home following a discharge.
