Through Support from the Innovation Grant Program, Catholic Charities Launches New Program

In February 2018, Catholic Charities of Tompkins/ Tioga was awarded funds to provide Health Outreach Coordination through Care Compass Network’s (CCN) third round of the Innovation Funds program.

With funding support from CCN, Catholic Charities of Tompkins/ Tioga launched their new program that will empower residents to improve their health and allow them to achieve their health goals. The Health Coaching program is targeted towards those that are uninsured or on Medicaid, particularly focusing on those in rural areas.

The Health Coaching Program works with individuals to develop goals, whether it’s finding transportation to get to the doctor’s office on a more frequent basis or help to quit smoking, whatever their personal goals are, Health Coaches are there to support them and link them to resources and services they need.

According to an article by Ithaca Voice, Health Coaches “walk with them, encouraging them to take one step at a time toward better health, helping them recognize the warning signs they shouldn’t ignore.”
