Primary Care Clinicians
Primary Care Clinicians are primary care givers of a patient that work in a hospital, skilled nursing facilities, clinics, or a patient’s home. Primary Care includes health promotion, disease prevention, health maintenance, counseling, patient education, diagnosis and treatment of acute and chronic illnesses, promoting effective communication with patients and caregivers that encourage the role of the patient to become a partner in their health care.
Job titles may include the following – Doctors, Doctor of Osteopathic, Nurse Practitioner, Physician Assistant, Podiatrist, etc.
