Care Compass Network Supports Partner Organizations Through IT Funding Opportunities

Care Compass Network (CCN) can help support partner organizations through approved IT funding opportunities. Currently, programs are in place to assist partners with new implementations of EMR/EHR systems and to provide funding to accommodate a RHIO and/or DSRIP requirements such as interfacing existing systems with a RHIO, customizing or upgrading an EMR to allow for partner reporting to CCN or to accommodate DSRIP project requirements, or hardware/infrastructure needs to bring organizations up to baseline to meet security requirements or improve workflow in conjunction with DSRIP projects or deliverables initiatives.

New programs are currently in development to provide platforms for Care Management and Telehealth. These will be communicated to the PPS once they are finalized.

If you have a need that is not met by any of the approved programs, but will have an impact on CCN’s DSRIP goals, please contact your Partner Relations Coordinator. Partner Relations will put you in contact with an IT Project Manager who will utilize IT Program criteria guidelines to help measure the impact of each request. The IT Program Criteria looks at how the request helps reduce avoidable hospital use (ED utilization, admissions, etc.), increases or enhances data sharing between care teams, transforms the delivery of healthcare, promotes community level collaboration, or support Value-based Payment transformation. Each request will be carefully considered to make sure that it has a sustainable use and is scalable/ expandable to other partner organizations within the PPS.
