Join us on Friday, June 11th, for an interactive and engaging presentation on building a community-driven network. Nationally recognized speaker, Karis Grounds, Vice President of Health and Community Impact at 211 San Diego Community Information Exchange (CIE), will be sharing some of the key components necessary to build a strong and sustainable CIE network. Karis will share how communities working within the CIE have experienced meaningful community impacts which allowed them to shift away from a reactive approach towards a proactive, person-centered care approach that focuses on inclusivity and health equity. Their CIE has brought partners together from across the continuum of care, creating one cohesive unit that utilizes a shared language, a database of community resources, and integrated technology platforms. These elements have promoted a cross-sector collaboration which is well positioned to deliver enhanced and high-quality community care.
The last part of the quarterly Stakeholders’ Meeting will be dedicated to CCN’s Strategic Plan. Mark Ropiecki will be sharing more major updates on the plan and advancements with the open and performance network concepts, along with a draft framework for a new program.
