Participating partners are given exclusive access to CCN’s Partner Share Point Portal where they can access important resources, meeting minutes, and materials that will help them in their efforts to achieve their project’s goals.  CCN is continually updating the portal, so please check back periodically for recent updates.

sFTP Site

Partners contracted with Care Compass Network with a signed External User Agreement are giving access to CCN’s sFTP Site, a secure location to upload monthly reports. *Disclaimer: For security purposes and to ensure that this service remains available to all users, we employ software programs to monitor traffic to identify unauthorized attempts to upload or change information, or otherwise cause damage. In the event of authorized law enforcement investigations, and pursuant to any required legal process, information from these sources may be used to help identify an individual.

 

CompassHub

The CompassHub was developed for partner organizations contracted with Care Compass Network. Those that have a signed External User Agreement are given access to the site.
