Cardiovascular Disease Management CVD – Project 3.b.i

Overview

Cardiovascular Disease Management ensures clinical practices in the community and ambulatory care settings use evidence-based strategies to improve management of cardiovascular disease. Strategies are focused on improving practitioner population management, adherence to evidence-based clinical treatment guidelines, and the adoption of activities that will increase patient self-efficacy and confidence in self-management.

Partner organizations that participated in this project selected a panel of patients who met the cardiovascular disease criteria and were then tasked with meeting a standard of care to positively impact these patients. Community-based organizations were brought in to conduct Chronic Disease Self-Management Program (CDSMP) courses which engaged and empowered participants to set and maintain self-management goals.
