Chronic Disease Management/ Prevention COPD – Project 4.b.i.i

Overview

The Chronic Obstructive Pulmonary Disease (COPD) project was designed to increase the number of New York residents who receive evidence-based preventative care and management for chronic disease. Based on the Community Needs Assessment done in 2015, Care Compass Network targeted the COPD population. CCN partners that participated in this project adopted early screening methods to identify patients that needed additional follow up or the provider felt that a pulmonary function test (PFT) would be a more effective diagnostic tool. The intent for management respiratory disease early was that it was believed it would help reduce avoidable readmissions and emergency room visits, therefore reducing costs.
