Community-Based Healthcare Navigation – Project 2.c.i

Overview

Health literacy, community values, language barriers, and lack of engagement with community health care services can result in avoidable use of hospital services. Those patients who do not understand how to access and properly use the health care system cannot be expected to use it effectively. Care Compass Network convened hospital and community-based organizations (CBOs_ to provide a variety of navigational services to Medicaid members throughout a 9-county region.

Through this project gains were made in the following areas:

  • Identified and addressed the social determinants of health that prohibit Medicaid members from utilizing appropriate health care services.
  • Development of strong, lasting partnerships between health systems and CBOs to close gaps in care.
  • Developed a care coordination model using efficient, consistent, and evidence-based protocols to ensure patients/ clients needs are addressed appropriated and navigated effectively.
